Lifestyle Management Programs - Reset Your Life
Enriched Health is an approved provider of Lifestyle Management Programs.
These programs are designed for people who are at risk of developing type II Diabetes. To be eligible for this program you must first complete the Australian Type 2 Diabetes Risk Assessment Tool and take it to your General Practitioner. Your GP can then provide you with a referral to see Enriched Health.
The Lifestyle Management program covers all areas of Diabetes and Pre Diabetes, Diet, Smoking, Stress Management and Physical activity.
The goal of these programs is to inspire healthy life changes and all sessions are conducted in a fun group environment.
Rebates are available from:
- Medicare for eligible participants (Bulk Billed for concession card holders)
- Private Health Insurance
- DVA
